Your Wellness Center membership includes the following:
• Towels
• Locker rooms with showers
• Dry sauna and whirlpool
• Indoor heated pool
• Aerobic classes
• Use of general fitness floor
• Orientation to fitness floor and equipment
• Express workout

Membership Availability: Membership at The Center is limited to Villages residents or renters, employees of The Villages and affiliate employees of The Villages age 18 or over.

Health History Questionnaire: All members, and potential members, must complete a Health History Questionnaire. A staff member will review your responses and a physician’s clearance may be required before participation.

Medical Clearance: The Villages Wellness Center follows the medical guidelines set by the American College of Sports Medicine, The American Association of Cardiovascular & Pulmonary Rehabilitation and The International Health and Racquet Sport Association. If you fall into one of our risk categories The Center will need medical clearance from your physician. The Center will handle this for you by faxing your physician our health history form.

Guest Privileges: Non-members are welcome to use The Center on a daily basis. Non-members must present a Villages I.D. or a Villages visitor I.D. A liability waiver must be completed and a daily pass must be purchased at the front desk. Guests may use all of the facilities with the exception to the MedFit area. Guests may purchase more than one daily pass at the time.

Youth Policy: A member may bring any youth, 16 years of age or older, as their guest. Any guest that is accompanying a member and not working out must stay in the front lobby area. The front desk staff will not be responsible for monitoring guests of members. All members are responsible for ensuring their guests follow The Center’s policies.

Billing: Your membership fee is due at sign up. The Center accepts the following forms of payment: check, cash, Mastercard, Visa or Discover.

Membership Renewal: Your membership is based on a 1-month, 3-month, 6-month or 12-month agreement. The Center will notify you in writing prior to expiration of your agreement. Please see Membership Services to discuss terms and conditions regarding specifics of your agreement. If you prefer to renew by telephone, please call 753-6910, Monday–Friday from 8:00a.m.–5:30p.m.

Medical Hold: The Center will temporarily place your membership on hold due to a medical situation for the following memberships: 6-month memberships: One (1) medical hold for up to 90 days per contract. 12-month memberships: Two (2) medical holds for up to 90 days per contract. 3-month Medfit/medfit lite memberships: One (1) medical hold for up to 30 days per contract. The Center must be notified as soon as possible when a member requires a medical hold. A physician’s note is required stating the start date and the end date of your medical hold. Until this note is recieved, member may not resume activities and no time adjustment will be given.

Membership Freeze: We realize that certain activities, such as vacations or personal time, come up during the year that will prohibit the use of The Center. To accommodate this, we offer a FREEZE TIME OPTION only on the following:
6 month contract - one (1) freeze for up to 30 days per contract.
12 month contract - two (2) freezes for up to 30 days each per contract.
You must inform the front desk or sign the freeze book at the front desk prior to going on freeze. Freezes will not be back dated. The freeze option must be utilized prior to your expiration date and does not carry over to the next contract. Time adjustment will be made to the expiration of your membership.

Cancellation Policy:
THREE-DAY CANCELLATION: If you wish to cancel this Membership Agreement, you must do so in writing to The Villages Wellness Center. The notice must be delivered in person or by certified mail, return receipt requested. You may cancel this transaction in writing any time prior to midnight of the third business day, exclusive of holidays and weekends, after the date of this transaction. Upon cancellation, in accordance with the above, The Center will make a full refund with such cancellation.
• TERMINATION OF MEMBERSHIP. The Membership Agreement may or shall be terminated as follows:
(A) MOVE. The membership shall be terminated if you cease to be a resident of the community generally known as The Villages. The Center will make a prorated refund to you of any prepaid dues within 30 days.
(B) DISABILITY OR DEATH. You may cancel this Membership if you become disabled, and your estate may cancel this Membership in the event of your death. You/your estate must prove any such disability or death by a doctor’s certificate or death certificate. The Center may require that you submit to a physical examination by a doctor agreeable to The Center and you to verify the
disability. The Center will make a prorated refund upon confirmation of your disability or death.
(C) CENTER RELOCATION OR CLOSURE.
The Center will refund the pro-rated cost of any unused services within 30 days if The Center relocates more than five (5) driving miles from its present location or ceases operation. You are advised to contact The Florida Department of Agriculture and Consumer Services within 60 days if The Center closes operation. The Center reserves the right to close temporarily for repair and renovation, not to exceed two (2) periods of seven (7) consecutive days in any calendar year, and upon sale for not more than fourteen (14) consecutive days, and your obligation for dues continues uninterrupted.
(D) END OF TERM. Immediately upon any expiration, cancellation, or termination of this membership, The Center requires that all outstanding charges must be paid and the membership card must be returned before any prorated prepaid membership dues are refunded.
(E) ANY PROPER CANCELLATION OF THIS AGREEMENT shall also terminate your obligation to any entity to whom the agreement has been assigned.
(F) UPON TERMINATION OF EMPLOYMENT from The Villages of Lake/Sumter, Inc. or any of its affiliates.
(G) EARLY CANCELLATION of agreement for reasons other than specified on reverse side of agreement will result in a prorated charge based on number of months used in membership. A 30 day cancellation notice is required.
